WHEN TO USE THE FORM: This form is coupled with and is responsive to Form 220.

HOW TO USE THIS FORM: If an appointment of a Personal Representative was not requested, delete all references to a Personal Representative.

COMMENTS: Under Nebraska Probate Code § 30-2418, if the Registrar is not satisfied that a Will is entitled to be probated in informal proceedings because of failure to meet the requirements of Nebraska Probate Code §§ 30-2416 and 30-2417 or any other reason, such Registrar may decline the application. A declination of informal probate is not an adjudication and does not preclude formal probate proceedings.
